Bike and Boat Day Camp Registration Request

• NO PEANUTS OR SHELLFISH
• No sharing of food among campers.
• Participation in recreational activities is mandatory.
• Swearing, bullying, and play fighting will NOT be tolerated. 
• Campers are responsible for all the items and equipment they bring to camp.

• No electronics are to be brought to camp.
• Campers that bring a cell phone must keep their cell phones in their bookbag/backpacks unless needed for an emergency.
• Yarmouth Recreation does not take any liability for lost, damaged, or stolen items.

• Provide your child with sunscreen (SPF 15+) labeled with your child’s name.
• Apply sunscreen to your child before they arrive at camp in the morning.
• Practice applying sunscreen with your child.
• Program leaders will monitor sunscreen application, and make sure there is plenty of time for reapplication during the day, however, we will not directly apply sunscreen to your child.
• Campers can use the buddy system with others if they need help applying sunscreen.
• Dress your child in a t-shirt or any shirt that covers their shoulders.
• Provide and encourage your child to wear a hat and sunglasses.

By checking this box, I understand that there is an inherent risk in participating in this program. I assume all risks and hazards incidental to the conduct of the activities taking place. I give permission to the event organizers to call an ambulance if the organizers determine it to be necessary. I hereby for myself, my heirs, executors, and administrators, waive and release all rights to claim for damages, injury or loss to me personally or my property against the event organizers and their agents, representatives, successors, volunteers and assigns, even if proven to be negligent.

I give my consent for staff at Yarmouth Recreation to seek medical emergency care for my child if necessary.

As a parent or guardian, I, give permission for (participant) to have his or her photo taken during Yarmouth Recreation activities. I also give permission to have their photo taken by the media (Eastlink, Vanguard, Yarmouth Recreation Social Media, etc..) during a Yarmouth Recreation activity.
